Reporting Concerns or Filing a Complaint

Patients or their authorized representatives who believe they have been subjected to discrimination, denied access, or treated unfairly may submit a complaint without fear of retaliation.

Concerns may be directed to clinic management at Advanced Orthopedic Pain Management & Wellness Center. Patients also have the right to file complaints with the appropriate government agencies, including:

• U.S. Department of Health and Human Services, Office for Civil Rights (OCR)
