Submit complaints or grievances without fear of retaliation
Be protected from the improper use, taking, or misappropriation of personal or private property by any clinic staff member, employee, volunteer, student, or contractor
Consent to or decline medical treatment, except in emergency situations, and withdraw consent prior to treatment initiation
Receive information regarding alternatives to medications or procedures, including potential risks and complications, except in emergencies
Be informed of the clinic’s policies related to advance healthcare directives and the patient grievance process
Provide consent prior to being photographed, except when photographs are required for identification or administrative purposes
Authorize the release of medical or financial information only with written consent, unless disclosure is otherwise permitted by law
Provide honest, accurate, and complete information related to their health and care
Inform clinic staff of current symptoms, medical history, hospitalizations, medications, and other relevant health information
Notify staff of any safety concerns, perceived risks, or issues related to care
Promptly report unexpected changes in condition to the appropriate healthcare personnel
Follow the agreed-upon treatment plan and care instructions
Ask questions when information is unclear or concerns arise
Understand the potential outcomes of treatment choices and the consequences of not following the recommended plan of care
Be aware of and cooperate with the healthcare professionals involved in their care
Treat staff and other patients with respect and consideration
Respect the privacy and confidentiality of fellow patients
Use clinic property responsibly
Fulfill financial obligations related to healthcare services in a timely manner
Patients or their authorized representatives who have concerns regarding patient rights, safety, quality of care, or clinic practices may submit a complaint or grievance without fear of retaliation. Concerns may be directed to clinic management or the designated compliance representative at Advanced Orthopedic Pain Management & Wellness Center.
Written complaints will be reviewed and addressed according to clinic policy. Patients also have the right to contact the appropriate state or regulatory health authority at any time.
